If you are an NDIS participant living in Greensborough, Montmorency, Briar Hill or Watsonia, Lotus Healthcare is here to provide in-home supports and other services as part of the delivery of your plan.
These include both non-medical services, such as companionship care and assistance with daily living activities, as well as nursing care, post hospital & transition care, and palliative care. We are also able to offer accommodation options and respite care.
Lotus Healthcare support workers are committed to providing highly personalised care for NDIS clients of all ages, in addition to Home Care Packages (HCP) and Targeted Care Packages (TCP).
We are a registered NDIS provider Greensborough participants and families can work in partnership with to receive a range of supports. These are designed to help clients live independently as far as possible, and include assistance for all kinds of daily living activities, including mobility and transport, as well as household tasks such as meal preparation and personal care.
All supports are tailored to the individual’s age, needs and circumstances (in accordance with their plan), and this flexibility means that our support workers can provide high levels of compassionate in home care Melbourne NDIS participants and families can benefit from.
Our team also consists of mental health allied support staff, as well as psychologists and registered mental health nurses, and so we are able to offer comprehensive care and support for all elements of participants’ health and wellbeing.
A key tenet of the NDIS is that it helps participants to build capacity in areas such as daily living, and social & community participation. In addition, we help clients to further build capacity with regard to choice & control through supporting their choice of service providers, and the ways in which the details of their plans are ultimately delivered.
In addition to our wide ranging experience as a service provider, we also assist people in making applications to become NDIS participants, and can guide you through all the necessary steps as you work towards getting an NDIS Plan approved.
Once you have a plan in place, we are here to provide advice on the full range and variety of services that are available to you, as well as assistance in finding the most accessible and appropriate providers for the services you are eligible for.
This is made easier by the fact the fact that over time we have been able to build up a substantial range of connections in Greensborough and the surrounding suburbs, and can put you in touch with a variety of support groups and social organisations, as well as doctors and other medical professionals who can provide services that are appropriate for your NDIS Plan.
Beyond the NDIS, we can also advise you as to any other funding that you might be eligible for, such as from private sources, the State Government, or under the auspices of the Commonwealth Home Support Programme (CHSP).
Lotus Healthcare provides a range of accommodation options so that participants do not need to stay in hospital for longer than necessary or be admitted to a Melbourne aged care facility when respite care is needed, or other more appropriate accommodation is not available. This includes both short term accommodation (STA) for between 2 to 6 weeks, as well as medium term accommodation (MTA) options.
We also operate a Day Centre in Yarrambat where both adults and children can enjoy disability care and support services in an inclusive sensory environment. Our trained and experienced support workers help NDIS participants to realise their individual development goals, as well as take part a variety of interactive and fun group activities.
In addition, as a local NDIS provider Greensborough allied heath professionals and medical practitioners are always on hand to provide all disability and health care services required.
