Our team of committed, compassionate support workers and medical professionals deliver exceptional personal care and services, as well as Home Care Packages (HCP) and Targeted Care Packages (TCP).
We work in close partnership with clients in the delivery of NDIS plans, and provide in-home support services for clients of all ages, including nursing services and palliative care, companionship care, as well as post hospital & transition care. A further key service that we provide is to help you to prepare your application to become an NDIS participant so that you can get your plan approved. In addition, we provide comprehensive NDIS plan management and support coordination services so that you feel fully informed as to what services are covered as part of your plan, as well as helping you to find and engage the best providers for those services.
We can also work with you to explore funding that you might be able to access in addition to the NDIS, such as State Government support or funding through the Commonwealth Home Support Programme (CHSP). There may also be private funding sources that are accessible to you.
As we are a local NDIS provider South Morang and Mernda participants working with us can also take advantage of our wide range of community contacts. This includes sports clubs, schools, libraries and support groups through which participants can take part in a range of social events. We are also able to help clients access local doctors and medical professionals when they are required.
Respite care is a valuable service that we provide for NDIS participants and their families. This aims to prevent people in need of care having to stay in hospital longer than necessary, or being admitted to a Melbourne aged care facility, through providing accommodation that is better suited to their needs.
We are able to provide both short term accommodation (STA) for between 2 to 6 weeks, and medium term accommodation (MTA) in Mernda and South Morang, while participants wait for more permanent solutions to be found.
In addition, support workers at our Day Centre in Yarrambat provide a variety of disability support services for both adults and children. These are tailored to individual needs, but there are also opportunities to participate in a range of group activities in a welcoming, sensory environment.
In addition, we partner with local South Morang and Yarrambat medical practitioners and allied heath professionals so that participants at the Day Centre are able to access personal care and disability support services in one place.
