We specialise in working with Preston NDIS participants of all ages in the delivery of their plans through providing a variety of in-home care and respite care, including nursing services, post hospital & transition care and palliative care. Our support workers also provide assistance for participants in a range of daily living activities and household tasks.
Lotus Healthcare medical professionals and support workers are committed, caring and compassionate, and provide disability care and in-home care services for NDIS clients of all ages, in addition to Home Care Packages (HCP) and Targeted Care Packages (TCP).
As a registered NDIS provider Preston participants regularly partner with our support workers in order to benefit from our services. Our aim is to provide the assistance clients need to live independently as far as possible, and so we can provide support to help you complete a range of daily living activities and household tasks, from shopping and meal preparation, to cleaning and laundry, to transport and participation in the wider community.
All our services are tailored to meet your specific needs and circumstances, which is why we are able to provide all-round, personalised home care Melbourne participants and their families benefit from. We also support clients’ mental health and wellbeing, and have psychologists, registered mental health nurses and mental health allied support staff as part of the Lotus healthcare team.
Helping NDIS participants to build capacity is an integral part of all the support services we provide, and through the delivery of their plans, clients are better equipped to exercise choice and control and to participate in the wider community.
Lotus Healthcare can also provide assistance when making an application to become an NDIS participant, advising you on what may be required in order to get approval for your plan. Following on from this, we will work with you to assess the full range of support services open to you, and which providers are best suited to your particular needs and circumstances.
We are also able to provide advice and guidance on other sorts of funding that you might be able to access, such as from the State Government or via the Commonwealth Home Support Programme (CHSP).
In addition, we act as an NDIS provider Preston wide and so have wide range of partnerships across Thornbury, Northcote and throughout the local community. We can therefore connect you with a network of support to meet all your needs, including with doctors and medical professionals, as well as support and sporting groups, schools, libraries and more.
The Lotus Healthcare Day Centre at Yarrambat provides a variety of disability care services suitable for both adults and children. Our highly trained support workers work with you to realise your individual development goals, while also providing opportunities to take part in a range of groups activities in a friendly and inclusive environment.
We also partner with local medical practitioners and allied health professionals so that clients using the Day Centre can easily access a range of medical and other support services.
In addition, we provide respite care that enables clients to avoid extended stays in hospital or having to be admitted to a Melbourne aged care facility, through providing access to more appropriate accommodation.
We are able to offer both short term (between 2 to 6 weeks) and medium term accommodation options, which are invaluable for NDIS participants awaiting more permanent solutions.
