Lotus Healthcare provides a range of in-home care and disability support services for NDIS participants in Heidelberg, Eaglemont, Rosanna and surrounding suburbs. We work with clients of all ages and their families, and the range of supports we offer also includes Home Care Packages (HCP) and Targeted Care Packages (TCP).
We work in close collaboration with participants and families in the delivery of NDIS Plans, and provide services such as companionship care and other forms of in-home care, as well as nursing services, post hospital & transition care, and palliative care. We are also able to offer a range of respite care and temporary accommodation options.
Our support encompasses a wide variety of services and, as a registered NDIS provider, Heidelberg participants and their families can access most of these in your own home. These include support for a range of different daily living activities and household tasks, such as shopping and meal preparation, cleaning, cooking, gardening and laundry, as well as personal care and other life skills.
In addition, we work closely with our clients to help them to live independently as far as possible, which is why we tailor our disability care and other supports to meet your particular needs and circumstances.
Our team of trained and caring support workers are committed to providing specialised and flexible in home care that delivers on NDIS participants’ plans, along with support for mental health and wellbeing provided by psychologists and other mental health allied support staff.
Our trained, professional support workers assist Heidelberg NDIS clients to build capacity in a variety of ways. This includes a greater ability to exercise choice and control through making decisions as to how plans are delivered, as well as enhanced opportunities to take part in social events and the wider community.
If you are planning on making an application to become an NDIS participant, we can provide advice and guidance about what is needed when you apply, as well as assistance in getting an NDIS plan approved.
Once this process has been successful, or if you already have an approved plan, we can then help you to understand the different sorts of services that are available and that you can access, as well as the best providers for those services.
As we are an established NDIS provider Heidelberg participants can also take advantage of the network of community supports we have established. These include support groups, sports and social clubs, libraries and schools, as well as local doctors and medical professionals who are on hand to provide care and treatment when required.
In addition to our NDIS plan management and support coordination services, we can also help you find out more about other potential sources of funds, such as the Commonwealth Home Support Programme (CHSP) or forms of State Government funding.
We are able to provide both short term accommodation (STA) for between 2 to 6 weeks, and medium term accommodation (MTA) options for NDIS participants. This can mean you avoid having to be admitted to a Melbourne aged care facility or undergo an extended stay in hospital, and can also serve as temporary accommodation while waiting for a permanent or more suitable arrangement to become available.
In addition, we have a Day Centre in Yarrambat where we are able to offer disability support services for both children and adults. This gives participants the opportunity to get assistance from our support workers to achieve their individual development goals, while also taking part in fun group activities in a friendly, sensory environment.
At our Day Centre, you are also able to access the services of local medical practitioners and other allied heath professionals, which means that getting the support and healthcare you need is always easy and convenient to do.
